Biography

Dr. Subramanian is a General internist and Geriatrician with advanced training in epidemiology and health services research from the Indiana University School of Medicine. She is considered an expert in Medical Home models for the Frail Elderly to address Geriatric syndromes such as Dementia to prevent functional decline and improve independence among older adults. 

Research Intrest

she has interests in developing and testing new healthcare models for frail older adults with chronic illnesses to improve health while lowering costs
